SSS Form R3 – Contribution List

The worksheet “R3”
is protected. You can write/input only
in the following items/captions:
EMPLOYER ID NUMBER
REGISTERED EMPLOYER NAME
TEL. NO. and ADDRESS
At page 1, item
no. 4 under the section “NAME OF MEMBER” there is a text box with text
***NOTHING
FOLLOWS***
Drag the text
box up or down accordingly.
You can
check/uncheck the check boxes in the section “ADJUSTMENT TYPE” even in page one
only. The checkboxes in other pages will likewise be checked or unchecked. If you want to use this form for household
employer, unprotect the sheet, under the section “TYPE OF EMPLOYER” >
“Regular” option, remove the fill (Right click > Format Shape…) of the oval
graphic object then apply fill in the oval provided for the “Household” option.
To unprotect the worksheet/s (MS Office Excel 2007), on the Review tab, in the Changes group, click Unprotect
Sheet.
In the
worksheet “Data” (also protected), just copy your records in the table provided
therein. Fill-in the necessary
information i.e. Quarter Ending; TR/SBR No.; Date Paid and Amount Paid. You can delete the contents of the cells but
don’t move, insert or delete columns and/or rows. Formulas in the R3 forms are linked to this
table.
A maximum of
225 records equivalent to 15 pages of the R3 are allowed in this “USER-FRIENDLY
FORM”.
Adding and
deleting record/s: First, unprotect the
worksheet “Data”. Type the new
employee/s’ information at the blank row of the table immediately following the
last record. Select cell D5, under the
column field “Surname”, then sort ascending.
In deleting separated employee/s, replace the employees' surname by "zz" (with out the quotation mark), select cell D5, sort
ascending, at the bottom, delete the record/s with surname zz (Warning: delete record or cell, not row).
